Systems Navigator - El Centro de la Raza

Description:

El Centro de la Raza is a voice and a hub for Seattle and Martin Luther King, Jr. County’s Latino community as we advocate on behalf of our people and work to achieve social justice. Through our comprehensive programs and services, we empower members of the Latino community as fully participating members of society. We also work to raise awareness with the public, and government, business and civic leaders about the needs of the Chicano/Latino community in the United States.

The Systems Navigator will help families learn to independently negotiate complex systems that provide needed services. These systems include but are not limited to City and County, education, employment, federal and state social services, health, immigration, law enforcement, and emergency response systems. At a minimum support includes; information about rights and responsibilities of families within systems, the purpose of systems, or what function why serve for families, and when and how systems should be utilized.

 

Skills/Eligibility:

ESSENTIAL FUNCTIONS

 

  • Design and implement a linguistically and culturally appropriate outreach plan, including outreach to El Centro de la Raza’s participants, Plaza Roberto Maestas and El Patio residents.
  • Plan, coordinate and deliver two group orientation workshops (identified as Needs Assessment topics) per month. Orientations will alternate between two sets of five areas of need. (Set 1 is employment, affordable housing, money management, transportation, and nutrition/food access. Set 2 is discrimination, healthcare coverage/medical services, childcare, the K-12 public school system, and college planning and access.)
  • Recruit participants of the group orientations to schedule follow-up one-on-one sessions.
  • Conduct one-on-one intake to assess specific needs and/or goals and create a plan with families.
  • Provide ongoing navigation support for families as they work their way through the appropriate identified systems.
  • Conduct regular follow-up with families until they have successfully completed their plan.
  • Meet contractual performance pay outcomes.
  • Track participant Impact Measures, follow the service model, track number of participants, sessions attended, hours involved, and number of activities during program enrollment.
  • Utilize the Systems Navigation Support Program Evaluation plan as a tool to collect and report program data.
  • Engage in partnerships and collaborations with other organizations.
  • Attend weekly meetings with Program Manager, Charlas and department staff meetings.
  • Provide program reports, monthly narratives, success stories and program performance report for the Board of Directors.
  • Refer qualified participants to El Centro’s other wraparound services, including Financial Empowerment and Immigrant Family Safety Planning.
  • Prepare end of year demographics report, site visit information, and other required documentation.
  • Enter program data into Salesforce database on a monthly basis.
  • Perform other duties and responsibilities as assigned.

MEASURABLE STANDARDS

 

  • Must be able to work in a multi-cultural/multi-ethnic environment.
  • Must be able to work with participants from a variety of ethnic and cultural backgrounds.
  • Must be able to maintain effective relationships with co-workers, participants, community representatives and the general public.
  • Must perform in a professional manner at all 
    • A. degree in a related field.
    • 5+ years of nonprofit experience or other relevant experience working in social services.
    • Strong classroom presentation skills
    • Bilingual Spanish/English both written and oral fluency.
    • Familiarity with Latino community, immigrant communities and/or diverse communities.
    • Strong computer skills in Microsoft Word, Excel and Outlook.
    • Strong organizational and interpersonal skills
    • Must have work experience in an office environment and adhere to office protocol.
