We are recruiting for the following position(s):
Full-time position available immediately for our Quality Improvement Department. The Colorectal Cancer Screening Patient Navigator will guide patients through each step in the process of obtaining a colonoscopy. The Navigator will use supportive communication to motivate and assist patient s to overcome obstacles to completing a colonoscopy. The Navigator will maintain on-going communication with patients, clinical staff and specialty care providers (Gastroenterologists and office staff), and community partners to smooth the patient’s transition from primary to specialty care. The Navigator must be able to work evenings and weekends as needed.
Qualifications, Education and/or Requirements:
- Must have a working knowledge of preventative health and its importance.
- Must be able to use good judgment, meet deadlines and prioritize workloads.
- Must be able to maintain professionalism and appropriate boundaries in all interactions.
- Must have good skills with Microsoft Office Suite (Excel, PowerPoint, Outlook, SharePoint, word, Acrobat Pro etc.)
- High School diploma or equivalent required.
- Minimum of two years of experience in nursing, medical assistant, or related clinical care delivery or community health outreach and education.
- Must be bilingual in English and Spanish.
