School of Public Health Academic Affairs Handbook

The Academic Affairs Handbook (or Faculty Handbook) contains information on procedures related to faculty hiring, retention, and promotion procedures.

Current Handbook Version

2008-2009 Academic Affairs Handbook, originally published July 3, 2007, updated August 24, 2009

We recommend that if you have an older paper copy of the Handbook you download and print the current version.

Revisions to Handbook

August 2009: Removed outdated offer letter guidelines from Appendix 3.

July 2009: Updated SPH Approval for Submission of a Grant through a Department Other than the PI's Primary Department Form.

June 2009: Added SPH-VA Memorandum of Understanding form.

June 2009: Updated Faculty Council section to reflect current Council governence by-laws.

May 2009: Updated Checklist D. The form version of Checklist D is offline until issues with Word 2007 can be resolved.

January 2009: Updated "Approval Form - Submission of a Grant Through Other than the PI’s Primary Department" (Appendix 17). Began transitioning references to SPH over to SPH.

December 2008: Added new "Submission of Grant through Other than the PI's Primary Department" policy and "Approval Form - Submission of a Grant Through Other than the PI’s Primary Department" (Appendix 17).

November 2008: Rolled dates forward to 2008-09. Updated link in Checklist G.

September 2008: Removed references to Faculty Applicant Flow Report; new process in place by Academic Human Resources to collect this data.

July 2008: Removed "courtesy appointment" terminology, corrected broken links

March 2008: Updated Appendix 3, Guidelines for Appointments and Promotions for UW School of Public Health (SPH) Faculty Based at External Institutions.

January 2008: Minor update of Dean's Office job titles.

November 2007: Revised Request to Open Faculty Search form.

September 2007: More clean-up. Restored missing pieces of Appendix 2.

August 2007: Cleaned up broken links in the document.

July 2007: 2007-08 version released. New SPH Policy on Managing Non-Grant and Contract Effort by Grant-Funded Faculty. Handbook now released as single PDF.
