Primary Responsibilities:

  • Create a positive experience and relationship with the member
  • Proactively engage the member to manage their care
  • Provide member education
  • Support transitions of care
  • Help to keep members compliant with their care plans 
  • Partner with care team (community, providers, internal staff)
  • Knowledge and continued learning of community cultures and values
  • May conduct member assessments if needed
  • Comprehensive care management

  • Referral to relevant community and social support services
  • Improve health and self – management of conditions
  • Provide transition and personalized care to help

Skills/Eligibility:

Required Qualifications:

  • Bachelor’s Degree (or higher) in social work, psychology, social services, human services, or behavioral sciences or Associate’s Degree (or higher) with Medical Assistant Certified in the state of WA or National Certification or High School Diploma / GED (or higher) with either an Indian Health Service (IHS), an LPN license in the state of WA or  equivalent work experience as a Community Health Worker (CHW)
  • Must reside in Pierce County, Lakewood, Tacoma, University Place or Puyallup
  • Must have resided within the local community for 1+ years
  • Knowledge of culture and values in the local community
  • Familiarity with the resources available within the community
  • Access to reliable transportation that will enable you to travel to client and / or patient sites within a designated area
  • Ability to create, copy, edit, send, and save documents, spreadsheets, correspondence in Microsoft Word, Microsoft Excel and Microsoft Outlook
  • Ability to travel locally within the county up to 60% of the time with mileage reimbursement (once cleared due to Covid – 19)

Preferred Qualifications:

  • Field Based experience
  • Bilingual fluency in English and Spanish
  • Experience working in Managed Care
  • Knowledge of WA Medicaid and Medicare populations
  • Experience in Counseling or Behavioral Health
  • Case management experience 
  • Experience working with members of all ages (children to geriatric)
