The Health Care Advocate works under the direction of the Population Health Coordinator in Seattle, Washington. The Health Care Advocate’s primary responsibility will be to implement an effort to increase breast cancer screening rates in Grays Harbor County. The position will work directly with patients and will implement a breast cancer screening effort including the following activities: assembling breast cancer mail outs to patients overdue for screening, reviewing electronic patient charts to identify those in need of screening, requesting records, sending detailed and timely patient communication, conducting live follow-up phone calls and fielding callbacks from automated calls. Must have attention to detail and organizational skills plus the ability to stay focused and schedule tasks that include large quantities of patient mailings and reminder calls.
