- If you or someone you know has experienced or is aware of possible discrimination, harassment, or sexual misconduct or if you have experienced any retaliation for reporting discrimination, harassment or sexual misconducting; requesting an accommodation, or participating in a process to resolve a report, please make a Civil Rights/Title IX Report
- If you suspect a financial or ethics activity that violates a law, regulation, requirement, or university policy, this should be reported at this toll free # 1-844-518-1420 or by using the ethics and fraudulent activity reporting form.

- Any concerns or complaints about research misconduct should be reported to the UW Office of Research Misconduct Proceedings.
