    Patient Navigator

    Scope

    Neighborhood House (NH) is a partner with International Community Health Services (ICHS) to provide health clinic services at Seattle World School, a middle and high school for newly arrived immigrant/refugee students in Seattle Public Schools. The Patient Navigator works with a team of two other Patient Navigators to provide interpretation and support services to medical, dental and behavioral health staff and to provide case management, linkage to support services, and health care navigation, for students and their family members. Drug and Alcohol prevention education for students and parents may also be conducted.

    Neighborhood House is hiring 1 individual to who is fluent in one or more the following languages to work with students and families (languages listed in order of preference most needed based upon the current student population): Spanish, K’iche’/Quiché,Somali, Tigrinya, Chinese/Cantonese, or Arabic.This position will be temporarily conducted by remote work and applicants will need to be proficient in using computers as well as contacting clients by phone, and video conferencing. Patient Navigators may also provide support at the school and by delivering food, medications or supplies to students home while practicing social distances and other safety precautions.  Once schools reopen the primary work location will be at Seattle World School. Patient Navigators are expected to work during clinic/school hours, generally Monday-Friday, 7:30 a.m.-3:30 p.m. Hours may vary during school closure times.  Once deemed safe to do so, in-person contacts with students and families at school, in the community or at their homes will be a key part of this work. As the health clinic is school based, staff will be expected to be strong contributors to the school team and culture.

    The position start date and hours are flexible.  Ideally we would like for someone to start in May or June for 30-40 hours per week.

    Responsibilities

    • Provide outreach (phone, telehealth, social media, home visits and other methods) to students and families to inform them of services available at the health clinic, invite them to events, or provide other information.
    • Assess student-family needs and make appropriate information or referrals to health, social service or other resources, as needed; follow agency protocols for referral follow up and quality.
    • Collaborate with medical and behavioral health providers to assess and respond to individual student needs related to wellness and school success.
    • Meet with families individually to provide information and coaching on common health issues and how they can support the wellness of their children.
    • Work with families (individually or in groups) to gather information (surveys, focus groups, etc.) to help inform clinic services.
    • Participate on student/health-centered teams to coordinate services and improve care, as needed. Provide insight and consultation to project partners on developing program options that are culturally responsive to the target populations.
    • Provide interpretation for students at the school based health clinic as needed.
    • Provide assistance, including interpretation, in preparing and delivering health education groups and prevention education workshops for youth and/or parents in coordination with ICHS.
    • Assist with clinic reception, registration and scheduling.
    • Review materials developed by ICHS or program partners for cultural appropriateness and meaning.
    • Maintain communication with team members, NH management and partners on student and community needs to help ensure program is responsive.
    • Assist team members with ongoing continuous quality improvement to enhance program operations, including problem solving, helping to develop program systems and procedures, or participating on team workgroups.
    • Maintain timely, accurate and complete records of all client activities in line with agency and funder requirements.
    • Liaison and bridge to key ethnic communities and/or leaders to help coordinate services.
    • Provide appropriate No Wrong Door Services to clients, including maximizing NH resources available for holistic, family centered services.
    • Perform other duties as assigned.
Skills/Eligibility:

Minimum Qualifications

  • AA in health related field, social work or education; experience accepted in lieu of degree. B.A. Preferred.
  • Bilingual (written and verbal) in English and one or more of the following languages: Spanish, K’iche’/Quiché, Somali, Tigrinya, Chinese/Cantonese, or Arabic.
  • Knowledge of, and experience working with English as Second Language cultural groups.
  • One year experience working in social work, patient-focused health care or education.
  • Knowledge of resources available to assist immigrant and refugee families, including financial/emergency resources, as well as health resources.
  • Team player who is resourceful, proactive, and comfortable working with diverse populations.
  • Good listening and oral/written communication skills, and a non-judgmental positive, problem solving attitude.
  • Demonstrated ability to perform job duties within the context of complex regulations and definitions, and willingness to comply with legal confidentiality requirements (HIPPA and FERPA).
  • Attention to detail and an ability to persistently, accurately and quickly complete extensive technical documentation.
  • Ability to excel in highly visible, highly public settings.
  • Computer literacy, including MS Office.
  • Provide organizational skills and ability to manage multiple tasks.
  • Willingness to comply with established agency policies and performance standards, which may include productivity/personal responsibility, client/customer service, teamwork and maintaining a professional demeanor.
  • Must have reliable, independent transportation for periodic travel to client homes and between sites.
  • Must be able to work a flexible schedule that includes some evenings and weekends.
  • First Aid Certification (within 30 days of hire).
  • CPR Certification (within 30 days of hire).
  • Sexual Harassment Training within 30 days of hire.
  • Child Abuse and Neglect training (within 2 weeks of hire).
  • Must pass Neighborhood House background check requirements.
  • Position requires Washington State bi-annual criminal background re-check and FBI fingerprint.

Desired Qualifications

  • Experience working with schools, parents, and middle school and high school age youth.
  • B.A. in health related field, social work or education.
  • Professional understanding of health provider options and systems.
