Community Health Case Manager II - Serving African American Communities - Neighborhood House
Education & Community Services, Regular, 40 hours per week, 12 months per year
Neighborhood House has a longstanding commitment to ensure that the most vulnerable of our community are connected to the services they need to be healthy and thriving. We are looking for a Community Health Case Manager (CHCM) to expand and enhance community based prevention services to homeless and high risk adult clients. CHCMs will provide Rapid HIV and Hepatitis C (HCV) counseling, testing and referral services, connecting marginalized and homeless populations with mental health and/or co-occurring disorders to substance abuse treatment, HIV/Hepatitis screening, behavioral health treatment and housing services. The priority population for these two programs will be serving African American/African and Latinx populations and services will include outreach, testing, case management and linkage to care.
The ideal candidate will have experience navigating complex systems, working in a dynamic and flexible atmosphere and have experience working with our target populations.

- Provide rapid HIV/HCV testing, counseling and referral to high risk (U.S. and Foreign Born) African American/African and Latinx adults
- Provide housing case management and assessment through King County Coordinated Entry system.
- Conduct comprehensive needs assessment and screening with clients to determine additional service or referral needs including mental health treatment, substance use treatment, domestic violence, homelessness, sexual risk factors, employment, education or other needs that arise.
- Make timely and appropriate referrals for treatment, health insurance coverage or other services.
- Maintain work standards to ensure compliance with HIPAA, Human Subject Protections, Case Management ethics and other agency guidelines.
- Evaluate client assessment data with healthcare professionals, mental health providers, nursing staff and others to appropriately meet the needs of the client
- Provide oversight of care being provided to assigned clients by phone or in-person and provide limited accompaniment to services in the community.
- Document all client-based activities and maintain confidential client records. Accurate data entry is required.
- Provide resources and compensation to clients per agency and grant guidelines including managing gift cards and bus tickets effectively and accurately.
- Attend community events to provide mobile outreach using an agency van, needs assessments, housing assessments and/or HIV/HCV testing and counseling. Some of these events may occur in the evening or weekends.
- Liaison to key ethnic communities and/or leaders to help NH coordinate services to underserved communities in the (US and Foreign Born) African American/African and Latinx communities of Seattle and King County.
- Provide direct referrals to community Mental Health and Medical partners and support prevention education at community locations to ensure saturation of targeted communities.
- Work as an active member of the Neighborhood House Project HANDLE team exhibiting professionalism and showing respect to all staff and clients.
- Attend all required meetings and trainings. Some may be evenings, weekends or multi-day conferences.
- Participate in clinical case reviews, training, and case consultations with Neighborhood House staff or other community partners as required.
- Perform other duties as assigned.
- Bachelor’s Degree in Social Work, Public Health or other related fields plus 3 years’ experience in case management and 3 years’ experience working in social work or human services, with an emphasis on low-income, high-risk and diverse populations, OR
- Associates Degree in any field with 5 years’ experience working in social work or human services, with an emphasis on low-income, high-risk and diverse populations may be considered with Director approval.
- Knowledge of and experience in HIV/Hepatitis programming, Mental Health and/or Substance Use Treatment, or working with people experiencing homelessness.
- Experience working with high-risk adult populations in King County
- Demonstrated ability to perform job duties within the context of complex Federal regulations and definitions and to complete extensive technical documentation.
- Self-starter, highly organized, dependable, strong problem-solving skills and be able to manage multiple complex problems
- Ability to communicate effectively, both orally and in writing, with individuals from a wide range of cultural backgrounds.
- Must be proficient in key computer applications (MS Office/Word/Excel/Access)
- Must have independent reliable transportation.
- Team player who is resourceful, proactive and comfortable working with diverse populations.
- Willingness to comply with established agency policies and performance standards, which may include productivity/personal responsibility, client/customer services, teamwork, and maintaining a professional demeanor.
- Must have reliable, independent transportation for frequent last-minute travel.
- Certification for HIV testing, counseling and certification (may be arranged after hire).
- Must be able to work a flexible schedule, that includes some evenings and weekends. Some out of state training will be required.
- Motivational Interviewing training (may be arranged after hire).
- First Aid & CPR Certification (within 30 days of hire).
- Sexual Harassment Training (within 30 days of hire).
- Child Abuse and Neglect Training (within 2 weeks of hire).
- Must pass Neighborhood House Background check requirements.
- Position requires Washington State bi-annual criminal background re-check.
- Master’s degree in Social Work or related field and one year of professional experience
- Experience with and knowledge of social determinants of health and impact on mental health and/or substance use
