The Community Health Programs Coordinator, under the direction of the Community Health Director, will be responsible for managing day-to-day administrative activities necessary for the successful delivery of our suite of secondary or tertiary prevention programs as defined by the YMCA of the USA’s Healthy Living Framework and Community Integrated Health (CIH) model.

ESSENTIAL FUNCTIONS:

  • Serves as the primary point of contact for individuals interested in participating in EBHI/Community Health Programs. Conducts initial callbacks; manages inquiries through Daxko Engage; Manages program specific email boxes. Assists individuals with registration into programs or orientation processes or refers to community partners for other services as needed.
  • Conducts general filing tasks and data entry: managing participant files with HIPAA compliance and assisting with recovery of documents as requested (by business office, Community Health Director, CFO, etc.); managing monthly volunteer service spreadsheet; completing timely REDCap data entry for LIVESTRONG at the YMCA and Y Weight Loss Program; managing Pathways to Wellness post program usage data for CHAS registry, etc.
  • Utilizing program volunteers as needed, prints, collates and assembles LIVESTRONG at the YMCA intake packets, WLP participant binders and program supplies, Pathways to Wellness participant files and curriculum, staff and volunteer training manuals, graduation programs and certificates, mailings, etc. Coordinates first and last day logistics for classes (including paperwork, branch and coach contacts, and preparation of participant resources).
  • Maintains inventory of program supplies (curriculum, T-shirts, graduation supplies, etc.), communicating purchasing needs to Community Health Director. Processes payments for LIVESTRONG alumni club, pulling and distributing shirts as required.
  • Coordinates Healthy Living Program Orientations (LIVESTRONG at the YMCA, Pathways to Wellness, Y Weight Loss Program, etc.) including recruitment/scheduling of necessary staff and volunteers and manages flow of these orientations. Schedules readiness interviews and make-up sessions for individuals who miss orientations (if and when space remains in classes). May also conduct some interviews or make-up sessions.
  • Works with Lifestyle Coaches and Community Health Director to ensure program space has been appropriately scheduled at branches (i.e., for LIVESTRONG at the YMCA pre and post program functional assessments, healthy living classes, LIVESTRONG and Pathways to Wellness alumni groups and specialty programs). Assists with finding community spaces (when needed) for program graduations, special events or classes.
  • Facilitates open communication, through the effective utilization of Listen First core skills, between Community Health Director, YMCA staff, volunteers, members, and community partners to create strong relationships and build ownership.
  • Takes a leadership role in supporting the success of Lifestyle Coaches and program operations volunteers (e.g., Coach’s Assistants and Peer Mentors), attending and assisting Community Health Director with monthly team meetings.
  • Assists with management of Healthy Living pages on YMCA website and coordination with marketing and communication department.
  • May represent the YMCA alongside (or sometimes in place of) the Community Health Director at health fairs or special community events where secondary and tertiary prevention programs are featured and/or in meetings or workgroups with various community partners. May also assist or take the lead in preparing and delivering community presentations and/or conducting community health detailing.
Skills/Eligibility:

QUALIFICATIONS:

  • Bachelor’s degree in Community Health, Public Health, Health Administration, Health Promotion or related field and 1 year of experience in office or program administration (or equivalent combination of education and work experience) is preferred.
  • Proficiency in MS Office programs such as Word, Excel, PowerPoint and Publisher. Strong written and oral communication, organizational and interpersonal skills as well as knowledge of time management techniques and basic accounting ability.
  • Emotional Maturity. The Community Health Programs Coordinator will be privy to the stories of individuals who have, in many cases, experienced significant trauma as it relates to their personal health and well-being. In service to these individuals, the Community Health Programs Coordinator must be able to appropriately manage his or her own emotional responses including managing the “righting reflex” and paying attention to signs and symptoms of empathy fatigue/vicarious trauma and seeking support for such as necessary.
  • Understanding of Behavior Change philosophy is required. Experience working with wellness, special populations or social service related functions is desired.
  • Training to be completed upon hire: CPR with AED, First Aid, Child Abuse Prevention, Harassment Training, Blood borne Pathogens, Listen First (Motivational Interviewing), Facilitating Change in Small Groups, Orientation to Healthy Living, Principles of Member Wellness, Engaging with Cancer Survivors, Developing Empathy for those with Excess Weight, HIPAA training, Trauma Informed Approach. Community Health Programs Coordinator will also complete (or audit) the facilitator/coach trainings for the programs they will support.
