Schedule and Worksite: This position works full-time Monday – Friday during standard business hours. HealthierHere is currently performing all work remotely during the pandemic. Some of the work may continue remotely on an ongoing basis, depending on business needs. There will be situations where the employee is required to report to the HealthierHere downtown office and/or an off-site HealthierHere work location. Employees will be provided with a HealthierHere-issued laptop and must maintain a workspace with an internet connection where they can reliably perform work and remain available and responsive during scheduled work hours. Employees must reside in – or be willing to move to - the Washington state Puget Sound region, and within a reasonable distance to the HealthierHere office in King County to respond to workplace reporting requirements.

Summary

The Integrated Care Specialist position reports directly to the Director of Clinical Practice Transformation and is responsible for overseeing and managing activities associated with the Washington Integrated Care Assessment (WA-ICA) - a collaboration between the Health Care Authority (HCA), the five Medicaid Managed Care Organizations (MCOs), and all nine of the Accountable Communities of Health (ACHs). The WA-ICA initiative supports a standardized process to assess level of integration for primary care and behavioral health practice sites across the state as well as supporting associated training, technical assistance, coaching and other activities that would assist providers in advancing integrated care within their organizations. The Integrated Care Specialist will support primary care and behavioral health providers in King County to participate in the WA-ICA and advance clinical integration within their organization. It will also work collaboratively with other ACHs, MCOs and the HCA to support the statewide rollout/implementation of the WA-ICA. 

The Integrated Care Specialist must be committed to: (1) utilizing an equity framework as a way of doing internal and external business and program development; (2) partnering with clinical and community partners, Tribes and relevant stakeholders to make system-wide improvements, and (3) facilitating collaborative and collective strategies that contribute to the elimination of health disparities and transform the health of the population in King County.

Major Responsibilities

Lead and oversee the statewide rollout and implementation of the WA-ICA in partnership with HCA, MCOs, ACHs, and the WA-ICA Workgroup

  • Convene, facilitate and project manage the WA-ICA workgroups and committees and related deliverables
  • Support the rollout/implementation of the WA-ICA tool, including training, data collection, and reporting
    • Ensure robust training of primary care and behavioral health providers across the state on the implementation of the WA-ICA tool, including updating training webinars, training materials, and holding office hours as needed during the data collection period
    • In partnership with the WA-ICA workgroup, develop a training for primary care and behavioral health providers on how to use the WA-ICA data and how to create a quality improvement plan
    • Develop, implement, and monitor WA-ICA project work-plan.

 

  • Provide programmatic oversight and work collaboratively with the HealthierHere data team on the WA-ICA data collection, analysis and reporting
    • Provide regular and consistent communication with primary care and behavioral health providers to ensure data collection timelines are met
    • Ensure data summaries and reports are developed according to timelines and shared with appropriate partners and stakeholders
    • Support the WA-ICA workgroup to provide presentations of data and strategize on use of data to support the statewide advancement of integrated care
    • Ensure appropriate agreements are in place to share data with appropriate stakeholders for quality improvement

 

  • Maintain regular and consistent communications with partners and key stakeholders
    • Coordinate with the HCA, MCOs, and ACHs, as well as partners to develop a shared vision for integrated, whole person care and a strategy to advance practices’ integration efforts
    • Communicate progress to HCA, MCOs, ACHs, and/or funders and related stakeholders.
    • Communicate progress to HealthierHere staff, Governing Board, and committees.
    • Complete and submit required reports in a timely fashion.

Lead and oversee HealthierHere’s local, King County, efforts to advance integrated care and support providers in the WA-ICA 

  • Convene and engage primary care and behavioral health providers, along with MCOs in King County to support the development of strategies to advance whole person integrated care
    • Coordinate and manage complex research and analysis, including environmental conditions, trends, policy, processes and research/ best practices to recommend solutions to advance integrated care.
  • Convene primary care and behavioral health providers in King County to determine barriers and challenges to implementing and advancing integrated care and elevate those challenges to support policy change
    • Analyze information gathered from partner meetings to develop clinical, community and Tribal- driven solutions to advance whole person integrated care.
    • Provide ongoing monitoring and process improvement for implemented programs.
    • Maintain feedback and accountability loop with partners.
  • Convene and coordinate with MCOs in the King County region to provide training, technical assistance, coaching and other supports to ensure alignment and reduce duplication
