Temporary, term-limited through June 30, 2023. Non-exempt employees are eligible for overtime. This position is grant funded. Potential to continue the position is dependent on availability of additional funding.

Schedule & Worksite:

This position primarily works full-time Monday – Friday during standard business hours. As needed, the employee will be asked to cover weekends, holidays, and after hours. If these coverage opportunities cannot be managed with flexible scheduling, hours in excess of 40 hours/week will be compensated with overtime pay (time and a half). In the event weekend and holiday coverage is needed it will be scheduled on a rotating basis with the rest of the Community Hub team.

All HealthierHere staff are currently performing all work remotely during the pandemic. Remote work may continue on an ongoing basis depending on business needs. There may be circumstances where this position is required to report to an offsite HealthierHere work location in King County. Employees will be provided with a HealthierHere-issued laptop and must maintain a workspace with an internet connection where they can reliably perform work and remain available and responsive during scheduled work hours (M-F from 8:00 a.m. to 5:00 p.m.). Employees must reside in the Washington state Puget Sound region, and within a reasonable distance to the HealthierHere office in King County to respond to workplace reporting requirements.

 

 

Summary

The Community Hub Coordinator reports directly to the Community Hub Manager and will work on a team under the Director of Clinical Practice Transformation. The Community Hub Coordinator will support the operations of care coordination services for the HealthierHere Community Hub. The main focus of the Community Hub Coordinator will be to support a network of care coordination partner agencies employing community health workers (CHWs) or other care coordination personnel. The Hub Coordinator will be supporting program operations and building system infrastructure for a new program, onboarding new network members, and leading efforts around training, technical assistance, and convenings. The Community Hub Coordinator will need to develop a strong familiarity with Care Connect hub functions, workflows, and services to be able to optimally support CHW staff as they navigate the program and online platform.

Care Connect Washington is a statewide COVID-19 care coordination model managed by the Washington State Department of Health (DOH) and implemented regionally. The HealthierHere Community Hub serves King County and is intended to identify and provide supports, using a low-barrier, human-centered, equitable model, for individuals and households needing to isolate and quarantine after testing positive for COVID-19 and throughout COVID-19 recovery. These supports will address health and social needs, reduce transmission, and mitigate the health, economic, and social effects COVID-19 has on communities, particularly those disproportionately impacted and historically marginalized. The HealthierHere Community Hub will be contracting with local community-based organizations to provide care coordination services, with referrals processed and assigned via the central Hub.

The Community Hub Coordinator must be committed to: (1) utilizing an equity framework as a way of conducting internal and external business and program work; (2) partnering with clinical and community partners, Tribes, and relevant stakeholders to make system-wide improvements, and (3) facilitating collaborative and collective strategies that contribute to the elimination of health disparities and transform the health of the population in King County.

Major Responsibilities

Primary:

  • Provide daily support to CHWs and CCA supervisors
    • Provide support and coaching to CCA supervisors whose CHWs who are not meeting performance metrics
    • Provide support to CHWs and CCA supervisors on Care Connect documentation in the Care Coordination Systems (CCS) Community Health Record
    • Support  CCA CHW staff in identifying and coordinating resources and services for Care Connect clients when needs arise outside of Care Connect services
    • Work with CCA supervisors to identify additional training needs
    • Manage the document library on the online platform
    • Assist the Community Hub Manager with creating and distributing supportive materials for CHW personnel and referral partners, including culturally responsive and relevant information and resources related to isolation, quarantine, and recovery for COVID-19 and other general health and community resources
  • Organize and facilitate CCA/CHW cross team convenings and trainings (in person and virtual)
    • Training planning done in response to CHW staff needs as identified by CCA supervisors
  • Support the Community Hub Manager with program operations, including development of processes, workflows, and systems to successfully manage a growing program
  • Process grocery orders for clients with appropriate follow-up to CHW staff
  • Manage the process for the delivery of food and care kits to CCAs
  • Support the processing of housing and financial assistance requests, including file management and initial review
  • Work with procurement staff to manage inventory of food and care kits
  • Promote a positive, supportive culture through effective communication, leadership, guidance, and support
  • Build effective working relationships with peers, leadership, and partners
  • Apply a health equity lens in all work and communication with internal and external partners

Secondary:

  • Coordinate and process referrals from multiple sources, including the WA State DOH, local medical clinics, community-based organizations, schools, and community members’ self-referral, etc.
  • Assign referrals to the appropriate CCAs and CHWs in a timely manner and according to multiple considerations including but not limited to cultural match, geography, language, and CHW caseload capacity
  • Maintain proper documentation when processing and assigning referrals in accordance with HealthierHere Care Connect policies and procedures
  • Assist in identifying and collecting client and CHW success stories for HealthierHere communications
  • Provide direct care coordination services to clients if needed during surges when CHW caseloads are full
  • Provide increased capacity and support to Care Connect during COVID surges in the King County region, including working a flexible schedule that might include some weekend or holiday hours depending on need
  • Deliver food and care kits to clients as needed on a rotating schedule
  • Perform other duties as assigned
