About: The Tubman Center for Health and Freedom is a community organization committed to the principles of healing and people’s liberation from systems that make us unwell. We work to advance health justice, culturally appropriate care, and integrative medicine. We are a collective of community members laying the groundwork for an innovative community health clinic that specializes in meeting the needs of marginalized communities in Seattle’s Puget Sound region. We are named after the hero Harriet Tubman, who concentrated on both freeing people from systems of oppression that threatened their health as well as providing clinical care for patients as a Nurse in the Union army. We follow this same methodology of addressing health and wellness from both systemic and clinical approaches.

Position Overview: Tubman Center for Health & Freedom offers a relational healthcare model that centers the needs and desires of our patients. Our Tubman Guides play an essential role in this process. Tubman Guides specialize in community to support clinicians, as well as work hand-in-hand with patients, to meet their health and wellness needs in and out of the exam room and clinical walls. 

This position involves acting as a patient supporter, navigator and advocate, while also performing duties to Black, Indigenous, LGBTQ and otherwise marginalized community members. 

This position will serve in our school-based health center (e.g. The Freedom Clinic) as well as our Healing House, where we are piloting our model of care with community members as we build our flagship center. All locations are in South Seattle. 

Strong candidates for this position are individuals who already are called to the role of the caretaker/fixer within their communities and families.

Responsibilities: 

• Offer patients relational support in the form of health education, systems navigation, community resource identification and enrollment assistance. 

• Provide case management/coordination. 

• Act as a liaison between community members and health, wellness and social service resources. 

• Advocate for community members within local agencies. 

• Ensure compliance with Tubman Health data collection. 

• Help plan and facilitate community meetings, training sessions, and programs that help advance the health of community members and patients. 

• Develop and maintain confidential client notes and records. 

• Collaborate and cross-function internally and externally. 

• Attend weekly Tubman Team Meetings, in-person as much as possible.

Qualifications: 

• Experience working in a healthcare setting or social support agency, preferably working with Black, Brown, Native, and otherwise marginalized clients with a focus on community-oriented care. 

• Lived experience considered. 

• Prior experience working across the lifelong age spectrum is preferred 

• At least 3 years’ experience working in/with Black and/or Indigenous communities. 

• Strong commitment to racial, social and health justice and patient advocacy 

• Ability to work collaboratively in a team-based healthcare environment.

Compensation: This is a full-time, salary position at $63,000. Excellent benefits! Medical, Dental, Vision for employee at 100% and dependents at 80%, $1,000 continuing education credit annually, five weeks of paid time off a year, on top of selected paid holidays, winter and summer breaks, and retirement benefits.
