Summary

The Lincoln County Health Department is committed to making a difference in providing public health services to those who live, work, play, worship and visit Lincoln County. This position reports to the Director of Community Collaboration and will work with a valuable team of health professionals and educators tasked with organizing and educating the community about best practices to improve health. This is an exempt position that works a Monday-Friday work schedule and on occasion a weekend or evening. 

This position performs duties that require discretion when preparing, disclosing, and handling information that is confidential, controversial, or sensitive in nature.  This role may communicate and maintain confidential records, be privy to information and documentation that require discretion including sensitive information with respect to employer-employee relations.  This role only requests access for relevant confidential information needed to perform this role and responsibilities.  

Program Activities

  • Work in life course programs including Maternal Child Health outreach, chronic disease prevention, oral health, injury prevention and access to care. 
  • Research and recommend proactive health initiatives that highlight population health practices that are designed to improve clinical outcomes of health services.
  • Work alongside the communications team to design informative, educational, and promotional materials and publications about mandated health laws, healthy life choices, and health practice.  ,
    • Develop programs that engage the community in managing health prevention and disease initiatives; assist in the delivery of organization-wide services during periods of public health emergencies and outbreaks.
    • Develop and enhance community collaborations to address health issues impacting Lincoln County. 
    • Identify engagement opportunities with community partners to address social determinants of health.
    • Represent Lincoln County at a variety of statewide and local meetings, serve on workgroups that address statewide and community health priorities.
    • Engage members of under-represented communities to identify program needs, outreach strategies, and techniques for engaging individual and community representatives in a shared vision of equitable service delivery.
    • Work with stakeholders, community groups, medical providers, elected officials, and local business groups in identifying outreach strategies and service deliverables that best benefit the overall health of the many communities served by the Lincoln County Health Department.
